1. Status of Financial Services Provider in terms of the FAIS Act

Fusion Outsourcing Services (Pty) Ltd is an authorised Financial Services Provider, registration number 2003/017437/07, FAIS license number 39433. Fabian Buchanan (ID 8204135115087), Ian Milne and Robyn Smith are key individuals approved by the Financial Services Board. Representatives of Fusion SA are appointed by virtue of a written mandate. 2. Remuneration

The appointment of representatives is in terms of a written contract of employment with Fusion SA. Fusion SA receives remuneration from product providers as per the maximum legislated commission permissible. The amounts of commission earned will be disclosed separately by each representative.

5. Independent Status of FSP and Professional Indemnity Insurance

Fusion SA has agreements with the following product providers: MS Life, a wholly owned division of Old Mutual and the Premier Group. In the past 12 months Fusion SA did not earn more than 30% of its income from any product provider. Fusion SA has no financial interest in any product supplier. Fusion SA carries professional indemnity insurance.

6. Authorisation

Fusion SA accepts responsibility for the actions of the above mentioned representatives acting within their listed mandates. Representatives are authorised to give advice and render intermediary services on product categories as enumerated on their independent statutory disclosures. Fusion SA is authorised to provide financial services in respect of the following licence categories:

1.1 Long-term insurance Category A
1.2 Short-term insurance Personal and Commercial Lines – Note: Fusion is a juristic representative of FSP 31724 which details appear on this link.
1.3 Long-term insurance Category B
1.4 Long-term insurance Category C

Fusion SA will not be held liable in terms of any prejudice in respect of services or advice provided by any representative which falls outside the scope of their authorisation. Any complaint in respect of any product which falls outside the definition of financial product of the FAIS Act cannot be forwarded to the FAIS Ombud. 7. Complaints Procedures

Please ensure that you retain proof of delivery where the complaint is delivered by hand or by any other means.

The financial services environment is complex. We will endeavour to address all reasonable requests from our clients, but may also refer you to a more appropriate facility. Where the complaint pertains to any aspect of our service, or any disclosures that ought to be made by us, we will endeavour to address those complaints in writing within five working days.

In instances where the complaint pertains to something not within our control, such as product information or investment performance, we will forward the complaint to the product provider concerned.
